Billing and Insurance

I am not In-Network with any insurance companies, however, I can provide Out-of-Network paperwork in order for you to receive reimbursement.  

Full payment for services are required at your visit. There is a significant reduction in my rate if you are not receiving paperwork for physical therapy reimbursement. This is known as a Wellness service/rate and can be billed to your HSA or Flex accounts.
